Become a model by following these
steps:
In the form to the right
send us your first name, last name, email
address, telephone number, your desired treatment and a list of any
medications that you are currently taking.
- There is a $50.00 minimum deposit
- A representative will contact you if you are selected
- A waiver will be signed before treatment and student settings will be applied
for safety
- Certified Instructor will be present during all
sessions
